While prescribing diabetic shoes, medicare and Medicaid have the following requirements; all insurance providers follow these guidelines. In order for Insurance to pay for the patient’s diabetic shoes and inserts, the physician must certify that they meet all of the following requirements:

  • Have diabetes
  • Have at least one of the following foot conditions:
    • Partial or complete amputation
    • Past foot ulcers
    • Calluses that could lead to foot ulcers
    • Nerve damage because of diabetes with signs of problems with calluses
    • Poor blood circulation or deformed foot
